本研究目的為探討結腸直腸癌老年病人術後困擾與憂鬱傾向的相關性及其影響因素。本研究採橫斷式相關性研究,自2019年5月1日至2020年3月7日,於北部某醫學中心二院區之大腸直腸外科門診收案,採立意取樣針對結腸直腸癌老年病人術後治療3至6個月,共86位結腸直腸癌老年病人參與研究,以「癌症病人困擾溫度計」及「老年憂鬱量表簡明版」收集資料。研究結果顯示結腸直腸癌老年病人術後困擾程度平均得分4.23分(SD=2.55),憂鬱程度平均得分為4.40分(SD=2.99),有無腸造口及術後有無接受化學治療者在術後困擾程度有顯著差異;癌症分期、有無腸造口及術後有無接受化學治療者在憂鬱程度具有顯著差異;而困擾溫度(DT)、記憶力/專注力問題、睡眠問題、擔心,以及對日常活動失去興趣,為結腸直腸癌老年病人術後憂鬱傾向之重要預測因子,可共同預測解釋憂鬱傾向67%之變異量。期盼透過本研究讓醫療照護者能關注結腸直腸癌老年病人的術後困擾及憂鬱傾向,主動提供心理支持。
The purpose of this study was to explore the relationships between postoperative distress and depressive tendency on elderly patients with colorectal cancer. The research subjects of this study were those elderly surgical patients with colorectal cancer who receiving radical surgery and related treatment for 3-6 months. A cross-sectional, correlational study was conducted with a purposive sampling of 86 elderly surgical patients with colorectal cancer from outpatients at a medical center in northern Taiwan. Data were collected with structured questionnaires including the Distress Thermometer (DT) and Geriatric Depression Scale Form (GDS-SF). The results of the study showed that the average postoperative distress score of elderly patients with colorectal cancer was 4.23 points (SD = 2.55), and the average score of depression was 4.40 points (SD = 2.99). There were significant differences in scores of the postoperative distress for colostomy and chemotherapy. And in scores of the depression, there were significant differences in cancer staging, colostomy, and chemotherapy. The most important predictive factors of depressive tendency of elderly patients with colorectal cancer were distress thermometer (DT), memory/concentration, sleep, worry and loss of interest in usual activities. The five predictive factors could be used to explain 67% of the variance. Through this study, expect that medical caregivers can pay attention to postoperative distress and depressive tendency on elderly surgical patients with colorectal cancer, and provide psychological support on their own initiative.
